Every agency touches the handoff. No one owns it.
A jail or hospital may know the release date. A community provider may own follow-up. A pharmacy sees the prescription. None of them sees the whole handoff, and the person leaving care is often expected to connect it.
When medication or follow-up falls through, people can end up back in the emergency department, crisis services, or jail.
